It is irreversible and too often results in amputation.<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>There are a lot of diabetics in Louisiana; they are both diagnosed and undiagnosed.\u00a0 Charcot arthropathy occurs after about ten years\u00a0or\u00a0so of no treatment or under-treatment.\u00a0 If one has diabetes, but never actually had it diagnosed, then it will be untreated, and Charcot can occur.\u00a0 Similarly, if one has under-treatment and is not optimized, then Charcot can happen as well.\u00a0\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>In order to prevent disability and\/or amputation, it is necessary to recognize a Charcot foot early.\u00a0 With that recognition must come treatment.\u00a0 It is thought that Charcot results from repetitive trauma to a neuropathic (insensate) foot over time.\u00a0 Half of patients will recall an injury to the foot that caused the problem, but half will not.\u00a0 Minor trauma can cause a neuropathic foot to develop Charcot\u2019s Arthropathy; even a small torque on the foot from errant gravel can do it.\u00a0 That being said, trauma is not mandatory to begin the cycle of fracturing.<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h3><\/h3>\n<h3 style=\"text-align: center;\">&#8220;Charcot Arthropathy will present with signs of inflammation.\u00a0 The foot will be large, swollen, red and warm.\u00a0 It may be deformed.&#8221;<\/h3>\n<p>&nbsp;<\/p>\n<p>It is often\u00a0misdiagnosed as infection or osteomyelitis.\u00a0 A red, hot and swollen foot with a standard white count and no fever is most likely Charcot.\u00a0 If there is doubt, as with feet with small ulcers or puncture wounds, a simple test can help.\u00a0 If the swelling and redness go away after 10 minutes of elevation, it is more likely to be Charcot. If not, suspect infection.\u00a0 One can have both of course.\u00a0 If there is an actual ulcer, probing can help.\u00a0 If a probe (sterile medical Q-tip) can touch bone through the ulcer, there is a 65% chance the bone is infected.\u00a0\u00a0For difficult cases, advanced imaging techniques will be necessary to distinguish Charcot from infection.\u00a0\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h3 style=\"text-align: center;\">&#8220;Neuropathy is an absence of normal sensation.\u00a0 A patient may not feel pinpricks, light touch or vibration.&#8221;<\/h3>\n<p>&nbsp;<\/p>\n<p>Your foot doctor or surgeon should have a medical-grade filament to test \u2018protective sensation\u2019 in the office.\u00a0 Neuropathy is a hallmark of Charcot. X-rays help a great deal for the diagnosis of Charcot.\u00a0 An experienced surgeon will be able to identify a neuropathic and\u00a0arthropathic\u00a0foot immediately.\u00a0 Certain regions of the foot are affected in certain manners.\u00a0 X-rays are also useful for the treating Charcot surgeon to evaluate stability and disease over time.\u00a0 We typically follow a patient for life with serial X-rays to make sure the problem does not progress.\u00a0 In addition, we recommend that a patient get new X-rays anytime there is even the smallest twist, crush or fall involving the foot\/ankle region.\u00a0\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>Bloodwork can help in the diagnosis and management of Charcot Arthropathy.\u00a0 We try to coordinate care with the endocrinologist and primary care physician about laboratory testing.\u00a0\u00a0<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>If you have diabetes and have for a while, you are at risk of this problem.\u00a0 If at any point, with or without trauma, you develop a swollen, red, warm foot you should be evaluated for Charcot.\u00a0 Only 50% of patients will have pain, so do not wait until you hurt to seek care.\u00a0 An experienced Charcot surgeon should see you, and that surgeon should know who the other members of your diabetes care team are.<span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><!--more--><\/p>\n<h3>FOR MORE INFORMATION ABOUT CHARCOT FOOT TREATMENTS,\u00a0CONTACT WARNER ORTHOPEDICS &amp; WELLNESS TODAY!<\/h3>\n<h4>ABOUT THE AUTHOR<\/h4>\n<p><strong>D<\/strong><strong>r.
